Information Needed for Registration

Parent or Guardian MUST Accompany the Student

  1. Proof of residency in the Lee School Zone:
         Electric, Water, or Gas Bill - Lease Agreement or Contract
  2. Current shot record
  3. Withdrawal documentation from previous school
    or institution and last report card.
  4. Copy of Birth Certificate
  5. Copy of Social Security Card
  6. Parents Drivers License
