Candidate Application and Information Packet

Attached is the following information we hope will assist you with the many forms you have to complete to become a candidate:
  1. Application for a Place on the Ballot   — after completion, please file this document with the personnel in the Superintendent’s Office. This application will need your signature, and your signature will need to be notarized. There are instructions with this form that will assist you.

  2. Eligibility Requirements for Public Office — These are the qualifications needed for a public office. [Board Policy BBA (Legal)]

  3. Code of Fair Campaign Practices (Form CFCP) — This is encouraged; however, subscription is voluntary. This may be filed upon submission of a campaign treasurer appointment form.

  4. Appointment of Campaign Treasurer by a Candidate (Form CTA) –This document must be filed with the above mentioned form, Application for a Place on the Ballot – Instruction Guide is included. This document will help you complete your CTA form.

  5. Amendment: Appointment of a Campaign Treasurer by Candidate (Form ACTA) – instruction guide is included.

  6. Candidate/Officeholder Campaign Finance Report (Form C/OH) — This document will need to be completed three different times, and you will need to file with the personnel in the Superintendent’s office. You will note that the first report is due 30 days before the election (Thursday, April 6, 2017), the second report is due 8 days before the election (Friday, April 28, 2017), and the final report (Form C/OH-FR) is due when you finalize your expenditures and contributions, etc.

  7. Candidate/Officeholder Campaign Finance Report Instruction Guide  — This is good information to assist you to complete the C/OH Reports.

  8. Candidate/Officeholder Report of Unexpended Contributions – instruction guide is included.

  9. Political Advertising, What You Need to Know — Additional information regarding what you can and cannot do regarding political advertising.

  10. Short Guide to Prohibition Against Using School District Resources for Political Advertising in Connection With An Election — This is good information.

  11. Title 15 Election Code — This document covers all the “rules and regulations” you need to know regarding running for a Board of Trustee position.

  12. Criminal History Search Consent Form for School Volunteers — this search consent form is a requirement for all school volunteers.
